Exam Forms

To access and complete exam forms:

1. Log in with your career account credentials at myPurdue. 
2. Under the “Academics” tab, select the “Graduate School Plan of Study” link in the “Graduate Students” box. 
3. Log in again with your career account credentials.

The option to schedule a preliminary or final exam will only appear after you have a submitted and approved plan of study on file. These forms are only needed by PhD and master’s thesis students.
