Checklist for Initial Indiana Licensure (REPA 3) through the Indiana Department of Education (IDOE)

Fall 2024

Please ensure all items below are completed before applying for licensure.

Licensure RequirementsCompleted
CPR/AED Certification
The IDOE has stipulated that the CPR/AED certification MUST include a skills/ hands-on demonstration (on-site, in a classroom). A certificate without this component will not be acceptable for licensure. Training cannot be entirely online. Visit the Licensure Website for a low-cost option provided in conjunction with the PUFD.
Suicide Prevention Training
The IDOE has mandated training MUST be for at least one hour. Any certification for training less than one hour will not be acceptable for licensure.
Content Test Scores
All passing Praxis scores must be on file with Purdue University (1631). Students must at least attempt the content test prior to Student Teaching, but a passing score is required for licensure.
Pedagogy Test Scores
All passing Praxis scores must be on file with Purdue University (1631). Students usually attempt the exam during Student Teaching. A passing score is required for licensure.
All program coursework and requirements have been successfully completed.
  • Student teaching grade is posted.Degree is posted on academic transcript (if applicable). NO LVIS360 application should be submitted prior to December 14, 2024, the last day of the semester; degrees are not anticipated to post until January 2, 2025.
Students in Career and Technical Education (CTE) program areas have had occupational clock hours verified by OTEL.

(Not required unless completing a licensure program in Career and Technical Education)
There are no holds or encumbrances on student records.

If all requirements outlined above can be successfully checked off, you are ready to apply for licensure! You can now proceed to the IDOE website via LVIS360 portal to apply for your license. Please remember your license will be dated as of the date of LVIS360 application submission and that, per IDOE mandate, your application will be denied if upon review we find any of these licensure requirements not met.
